**** Please print out and fill in the appropriate forms before coming to the charter. If you will be diving several days in succession, just fill out one copy of the appropriate form using the date range of up to three days.****

Travel and Excursions Liability Release

Print out and fill in this form for all dives. Use the following for the Trip Operator field - Divers Wearhouse, Riverside Dive Resort, Under Pressure Dive Charters

Boat Travel Liability

Print and fill out this form for all boat trips. Complete all fields with the exception of the boat name. This can be filled in at the boat.

Thank You! Divers Wearhouse would like to thank all of you for diving with us in 2009 and we look forward to seeing you again next summer out on the open water.

Just because the warm weather has left us, we aren't slowing down. We will soon be open seven days a week to serve you, so drop by and see us. We are also planning more warm water trips , so watch the web site for that information as well as all of the courses like EFR, Nitrox and Equipment specialist that we will be holding throughout the winter.

And for those brave folks, we may even get in a few ice dives if there is enough interest!

So drop by the shop this winter to check out the new gear, and bring in your gear for it's annual service early and beat the rush for next year!
